STATE OF THE ART AND EFFICIENCY OF ROBOTIC THERAPY IN UPPER LIMB REHABILITATION – A LITERATURE REVIEW

ABSTRACT

Stroke represents the main cause of disability in adults. The disability is caused by motor or sensitive impairment, loss of interjoint coordination, spasticity and pathological synergies that occur after stroke. The new directions in motor rehabilitation point at facilitating neuroplasticity by multimodal stimulation, like virtual reality, or by active assistance in task specific training, like rehabilitation robots associated or not with functional electrical stimulation (FES). The aim of this article is, on one hand, to analize the way in which the existing robots face the biomechanical chalenges of modeling the upper limb's movements and, on the other hand, to evaluate the efficiency of robotics in rehabilitation, by pointing out the results of the existing clinical trials. Finally, possible directions for future research are discussed.

BRAINSTEM AUDITORY EVOKED POTENTIALS IN PARKINSON’S DISEASE

ABSTRACT

Background. Parkinson’s disease is a neurodegenerative disorder caused by loss of dopaminergic neurons in the substantia nigra, but also in other dopaminergic and nondopaminergic areas of the brain and mainly in the brainstem. Auditory evoked potentials are routinely used in clinical practice to evaluate the function of the auditory nerve and auditory pathways in the brainstem. The aim of this study was to investigate the auditory brainstem pathways in patients with Parkinson disease.

Materials and methods. In this study was included 34 patients with Parkinson’s disease. The control group was composed of 29 healthy age- and sex-matched subjects. Detailed examination were performed in all individuals and the parkinsonian patients were stage between 2 and 4 according to Hoehn and Yahr’s classication. Recordings of BAEPs were performed with Nihon Kohden Neuropack using 80 dB HL alternating polarity clicks in each ear at a rate of 10 s-1.. Averaged potentials to 1,000 clicks were obtained.

Results. The BAEP results were interpreted for the latencies of waves I, II, III, IV, V and Interpeak Latencies (IPL) I-III, III-V and I-V. The results of our study have shown that the waves II, III, IV, V and IPL III-V were significantly delayed bilaterally. This modifications does not correlates with the age or duration of disease

Conclusion. This study showed some modifications of auditory evoked potentials wich can be determined by the neurodegenerative process that affects the brainstem.

ANKYLOSING SPONDYLITIS AND MULTIPLE SCLEROSIS: A SURPRISING PARALLEL

ABSTRACT

Ankylosing Spondylitis (AS) and Multiple Sclerosis (MS) are two different chronic autoimmune and inflammatory diseases, one rheumatic and the other one neurological, apparently without any connection between them. Yet a few case reports proved that AS may be associated with MS and this situation should be considered in some epidemiological environment.

Although the geographic map of incidence is approximately superposed for both entities there are only a few cases of association. Recent studies revealed that most loci associated with MS are located very close to or even within genes encoding molecules involved in immune system function, confirming the hypothesis that MS is a disease with immunologic mechanism. In addition, some of these loci are also associated at least with another autoimmune disease, thus reinforcing the idea that autoimmune diseases may have a common origin.

If MS is more common in patients with AS than in the general population, we can speculate the existence of common factors involved in these diseases. Because of the limited number of cases that associated these two diseases, the correct diagnosis of both entities considered separately or together becomes mandatory especially reflecting on the anti-TNFα agents used to treat AS which may exacerbate or induce a latent MS or demyelination processes in the central nervous system. In terms of pathogenic mechanisms, they are poorly known in both diseases, but each is characterized by an initial immune and inflammatory attack, in MS the target is the myelin sheath and in AS the enthesis tissue. A case with association between the two diseases raises important issues in choosing an appropriate treatment.

SOCIO-ECONOMIC ISSUES IN ALZHEIMER’S DISEASE. RESILIENCE AND ALZHEIMER’S DISEASE CAREGIVERS

ABSTRACT

Alzheimer’s Disease (AD) is the most common form of dementia in the old population, carrying a major negative influence upon society and caregivers, as its socio-economic implications are enormous. The devastating impact of AD is also due to the fact that the diagnosis is difficult to be established in an early stage, and in spite of the advances made in medicine, the new symptomatic and disease modifying therapies have still only a mild though promising positive effect. Caregivers are exposed to increased risk of disease themselves, due to the high burden which predisposes them to stress related psychological, behavioral or cognitive disorders. Greater caregiver resilience can predict lower levels of depression. Also a strong external support system was associated with increased resilience. Establishing a pattern for the behavioral and psychological manifestations in dementia can have as direct effect developing better coping strategies in order to reduce stress and the burden of the caregivers.

Some data regarding the economic and social impact of AD, as well as facts and numbers from our local centers will be described in the following pages.

IS THERE A LINK BETWEEN A LOW LEVEL OF VITAMIN D AND MULTIPLE SCLEROSIS?

ABSTRACT

Multiple Sclerosis is a neurological disorder consisting in autoimmune and inflammatory manifestations. Its evolution is characterized by axonal demyelination among the central nervous system.

A growing body of evidence suggests lately that vitamin D deficiency is a very common condition in Multiple Sclerosis. On the other hand, during the last years, several studies have shown a correlation between high vitamin D serum levels and a decreased risk either for developing this illness, or preventing its evolution towards disability. Vitamin D is currently being considered an important environmental factor for this disease, therefore being widely debated whether its supplementation would actually be benefic, and if yes, what results can be expected in such a therapeutic approach.

Our brief look upon some of the latest results in the field of research for this matter is intended to synthesize what is known at this moment and what future directions would be.
